Background Check Forms
As part of the licensing process, you will need to complete two Iowa background check forms. All individuals living in your home that are age 14 or older need to complete the Iowa Criminal History Record Check and Child Abuse Record Check forms - and - an out-of-state record check form if they've lived out of state within the last five years. This includes individuals that stay in the home on a regular basis (such as adult children that are college students or other relatives) or a part-time basis due to shared custody.
